Abstract: Introduction: Acute appendicitis is one of the most common surgical emergencies, particularly in the pediatric population. Prompt and accurate diagnosis is crucial to avoid complications such as perforation and abscess formation. Objectives: The main objective of the study is to find the diagnostic value of CRP and TLC in acute appendicitis. Material and methods: This study employed a retrospective observational design and conducted at CMH Muzaffarabad from January 2023 to January 2024 to assess the diagnostic value of C-reactive protein (CRP) and total leukocyte count (TLC) in acute appendicitis. The study included patients who presented to the hospital with clinical suspicion of acute appendicitis between a specified time period. Inclusion criteria comprised patients of all ages who underwent blood tests, including CRP and TLC, and subsequently underwent surgical exploration or had a confirmed diagnosis of acute appendicitis through other diagnostic modalities. Results: A total of 210 patients with clinical suspicion of acute appendicitis were included. All patients underwent blood tests, including C-reactive protein (CRP) and total leukocyte count (TLC). Subsequently, the diagnosis of acute appendicitis was confirmed through surgical exploration or other diagnostic modalities. The mean CRP level in the study population was found to be 25.4 mg/L, with a standard deviation of 12.8 mg/L. The median CRP level was 23.5 mg/L, and the interquartile range was 17.2-31.8 mg/L. Conclusion: In conclusion, the study on the diagnostic value of C-reactive protein (CRP) and total leukocyte count (TLC) in acute appendicitis provides valuable insights. The results suggest that both CRP and TLC have potential as diagnostic markers for acute appendicitis. The study findings indicate that elevated levels of CRP and TLC were observed in patients with acute appendicitis compared to those without the condition.
